Pursuant to TX House Bill 4224 Section 181.105 of the Texas Health and Safety Code, please be advised of the following:

1.     To request access to health care records:

·       Contact the office coordinator at 512-399-4276

·       Submit a request via phone call or in person

·       Records will be provided within 15 days of request
